1. Eligibility:
- This gift card is redeemable only by individuals aged 18 years and older.
- The gift card is non-transferable and can only be used by the person whose name appears on the gift card.
- Redemption of this gift card is subject to a medical consultation and the final decision of a certified medical practitioner
2. Gift card Validity:
- This gift card is valid for 6 months from the date of issue.
- The gift card must be presented at the time of booking or during the appointment.
3. Appointment Booking:
- Gift card holders must contact The Cooper Clinic to schedule their vasectomy procedure.
- All bookings are subject to availability and clinic operating hours.
- The gift card must be presented at the time of booking or during the appointment.
4. Exclusions:
- This gift card is only valid for vasectomy procedures. Extra fees may occur for additional services (such as misplaced testing kits that need to be sent to the holder of the gift card)
- Any costs incurred outside of the standard procedure (e.g., medication, additional consultations, pre-existing medical conditions) will be the responsibility of the patient.
5. Refunds & Exchanges:
- This gift card is non-refundable, non-replaceable, and cannot be exchanged for cash or any other services.
- If the medical practitioner deems the procedure medically inappropriate for the gift card holder, the gift card cannot be refunded.
6. Rescheduling & Cancellations:
- If the gift card holder needs to reschedule or cancel their appointment, the terms and conditions as found on www.thecooperclinic.co.nz apply
- Failure to attend a scheduled appointment without prior notice may result in the forfeiture of the gift card and the full price of the procedure will be invoiced to the holder of the gift card.
7. Limitation of Liability:
- The Cooper Clinic is not responsible for any loss, damage, or injury arising from the use or misuse of this gift card.
- The clinic reserves the right to modify or terminate the gift card program at any time without prior notice.
